Right of refusal When you exercise your right of refusal, you may : refuse to have all of your health information contained in the DSQ information banks disclosed through the DSQ refuse to have all of your health information collected prior to a date you determine disclosed through the DSQ; Change your refusal; rescind your refusal. The only way to communicate your desire to have your health information disclosed through the DSQ is to file a formal request to exercise your right to opt-out. You may, at any time, refuse to have your health information in the DSQ information banks released to authorized providers or agencies. However, your health information will continue to be collected in the DSQ information banks. This collection will ensure, among other things, that your health information is complete if you decide to withdraw your refusal. Even if you have exercised your right to refuse, relevant health information in the area of imaging (X-ray, ultrasound, MRI, etc.) may be communicated to medical specialists when interpreting a new examination. In no case will your refusal affect your right to receive the care and services required by your health condition. The only way to communicate your wish that your health information not be released through the DSQ is to file a formal request to exercise your right to refuse. Right to Refuse for Minors and Incompetent Adults The refusal of a child under 14 years of age or a person who lacks capacity must be expressed by the person’s parent, guardian, tutor or mandatary.
