The member must ensure that all fields are completed truthfully and accurately.<\/p><p data-start=\"711\" data-end=\"1036\"><span style=\"text-decoration: underline;\"><strong>Profession \/ Accredited Modality<\/strong><\/span><br data-start=\"743\" data-end=\"746\" \/>The member must indicate the profession under which the service was provided. Only CPMDQ-recognized modalities for which the member is accredited may be selected or written. Examples include Naturopathy, Homeopathy, Massage Therapy, Kinesitherapy, Orthotherapy, Osteopathy, and Kinesiology.<\/p><p data-start=\"1038\" data-end=\"1089\"><em><strong>Member Identification<\/strong><\/em><br data-start=\"1059\" data-end=\"1062\" \/>The receipt must include:<\/p><ul><li>the member\u2019s full name<\/li><li>the CPMDQ license number<\/li><li>confirmation that the member is in good standing<\/li><\/ul><p data-start=\"1198\" data-end=\"1302\"><span style=\"text-decoration: underline;\"><strong>Policyholder Information<\/strong><\/span><br data-start=\"1222\" data-end=\"1225\" \/>All required fields relating to the insurance policyholder must be completed.<\/p><p data-start=\"1304\" data-end=\"1467\"><span style=\"text-decoration: underline;\"><strong>Patient Information<\/strong><\/span><br data-start=\"1323\" data-end=\"1326\" \/>All required patient-related fields must be completed. The patient may be the policyholder or another insured person listed under the policy.<\/p><p data-start=\"1469\" data-end=\"1534\"><span style=\"text-decoration: underline;\"><strong>Date and Cost of Consultation<\/strong><\/span><br data-start=\"1498\" data-end=\"1501\" \/>Each consultation must include:<\/p><ul><li data-start=\"1469\" data-end=\"1534\">the date of service<\/li><li data-start=\"1469\" data-end=\"1534\">the service fee<\/li><li data-start=\"1469\" data-end=\"1534\">the total amount charged before taxes<\/li><\/ul><p data-start=\"1622\" data-end=\"1710\">The amount indicated for insurance purposes must always be the service fee before taxes.<\/p><p data-start=\"1712\" data-end=\"1887\"><span style=\"text-decoration: underline;\"><strong>Description of Service<\/strong><\/span><br data-start=\"1734\" data-end=\"1737\" \/>The receipt must clearly state the exact nature of the service provided, according to the accredited modality under which the treatment was performed.<\/p><p data-start=\"1889\" data-end=\"2017\"><span style=\"text-decoration: underline;\"><strong>Therapist\u2019s Signature<\/strong><\/span><br data-start=\"1910\" data-end=\"1913\" \/>Each receipt must be signed by the member. A handwritten or legally valid digital signature may be used.<\/p><p data-start=\"2019\" data-end=\"2060\"><span style=\"text-decoration: underline;\"><strong>Prohibited Practices<\/strong><\/span><br data-start=\"2039\" data-end=\"2042\" \/>Members may not:<\/p><ul><li data-start=\"2019\" data-end=\"2060\">leave required fields incomplete\u00a0<\/li><li data-start=\"2019\" data-end=\"2060\">alter or modify any CPMDQ receipt format<\/li><li data-start=\"2019\" data-end=\"2060\">issue receipts to family members<\/li><li data-start=\"2019\" data-end=\"2060\">falsify any information<\/li><li data-start=\"2019\" data-end=\"2060\">issue receipts for services not rendered or under modalities they are not accredited for<\/li><\/ul><p data-start=\"2210\" data-end=\"2298\"><strong>FREQUENTLY ASKED QUESTIONS<\/strong><\/p><p data-start=\"2328\" data-end=\"2605\"><span style=\"text-decoration: underline;\"><strong>UNDER WHAT MODALITY CAN I ISSUE INSURANCE RECEIPTS?<\/strong><\/span><br data-start=\"2379\" data-end=\"2382\" \/>You may issue insurance receipts only under the modalities for which you are officially accredited by the CPMDQ. You may issue receipts under one or several modalities only if you hold the corresponding CPMDQ accreditation.<\/p><p data-start=\"2607\" data-end=\"2857\"><span style=\"text-decoration: underline;\"><strong>SHOULD I INCLUDE TAXES ON MY INSURANCE RECEIPTS?<\/strong><\/span><br data-start=\"2655\" data-end=\"2658\" \/>No. Taxes must never be included on CPMDQ insurance receipts. The amount indicated must always reflect the service fee before taxes. Insurance receipts are insurance declarations, not sales receipts.<\/p><p data-start=\"2859\" data-end=\"3071\"><span style=\"text-decoration: underline;\"><strong>CAN I ISSUE AN INSURANCE RECEIPT FOR A GIFT CERTIFICATE?<\/strong><\/span><br data-start=\"2915\" data-end=\"2918\" \/>No. Insurance receipts cannot be issued for gift certificates, whether purchased or given. Gift certificates are not reimbursable by insurance companies.<\/p><p data-start=\"3073\" data-end=\"3434\"><span style=\"text-decoration: underline;\"><strong>CAN I ISSUE AN INSURANCE RECEIPT FOR A FAMILY MEMBER?<\/strong><\/span><br data-start=\"3126\" data-end=\"3129\" \/>No. Insurance companies prohibit the issuance of insurance receipts to immediate family members or to anyone living in the same household. \u201cImmediate family\u201d includes the member\u2019s spouse, children, parents, brothers, sisters, parents-in-law, grandparents, sons-in-law, daughters-in-law, and grandchildren.<\/p><p data-start=\"3436\" data-end=\"3697\"><span style=\"text-decoration: underline;\"><strong>AM I AUTHORIZED TO ISSUE INSURANCE RECEIPTS AS A CPMDQ MEMBER?<\/strong><\/span><br data-start=\"3498\" data-end=\"3501\" \/>Yes. CPMDQ members in good standing may issue insurance receipts to their clients.
